Homophone. Similar to the original clue. Not sure how “rhythmic” points to homophone.
LAT3: Holds on to top of respirator while having trouble breathing (6)
CH. Top or bottom or somewhere in the middle. Not strictly the same part of speech for breathing.

Your comment Seems it's i it needs, not a, but maybe you're using both as numeral 1. Also needs a container indicator.
My response: The "container indicator" is "about", with "run" sufficing for "race". I agree that "...lacks one desperate…" would have been better than "..lacks a desperate".
